A near miss is one box: what nearly happened. The site fills itself in, and a location and a time are optional. With no signal, the phone keeps the report and sends it once, when it can. The fewer questions you ask, the more you hear.

The managers who cover that site get a push to their phone, a red card on the app's home screen and a bar across every page on the web. It cannot be muted and it stays until somebody presses Seen it, so a report never sits unread in an inbox.

Add notes, say who it is with, raise a job to fix the cause, and close it with an outcome. On Pro you can go further: rate how bad it could have been against how likely it was, record why it happened, set actions with owners and due dates, send the report as a PDF to whoever needs it, note whether HSE was told, and tell the person who reported it what changed.

Issues, snags and repairs in the same place

For things that are broken rather than dangerous, staff raise an issue with a photo from their phone. It gets a priority, an owner and a due date, moves through to fixed, and can go to a reviewer for sign-off before it is done. The person who raised it hears when it is fixed, and the person it is with is reminded before it is overdue.

What is a near miss?

Something that could have hurt someone or damaged something but did not: a pallet that slipped, a cable across a walkway, a step that gave. Reporting them is how you fix the cause before the day it does hurt someone.

How do staff report a near miss?

From the staff app on their phone, or from the web on any device. They say what nearly happened, and the site is already filled in. If there is no signal, the phone holds the report and sends it when it can.

Can near misses be reported anonymously?

No. Reports carry the reporter's name, so a manager can ask a follow-up question and tell them what was done about it. If anonymity matters more than that in your workplace, this is worth knowing before you choose.

Can staff add a photo?

On the web, yes. In the phone app a near miss is words only for now, while an issue from the phone can carry a photo.

Does it submit RIDDOR reports?

No. On Pro you can record whether HSE was told, when and how, so the investigation shows it, but the report itself is made on HSE's own form.

What is the difference between an issue and a near miss?

A near miss is about safety: something that nearly caused harm. An issue is a problem to fix: a broken door, a leak, a snag on site. Both are raised from the phone; an issue becomes a job with an owner, a priority and a finish.
